If an uploaded image file is dangled (e.g., when the user edits or deletes the event, or when the user uploads the image, but the whole form doesn't pass the validation). The image file should be deleted.

In the latter case, another possible solution is to upload the image to a temp folder first, and move it to the main folder once it's ready
